Survey for families of children who are receiving a service from a CDNT, or who are on a waiting list for a service from a CDNT
We have launched a review of the operation of the Children’s Disability Network Team (CDNT) service model, on behalf of the HSE.
In part 1 of this review, we would like to hear the experiences and opinions of families with children who are receiving a CDNT service or who are on a waiting list for a service from a CDNT.
Our family survey was first distributed via CDNTs. However, we know the survey was not received by all parents, so we then distributed it directly through social media, disability organisations and other relevant stakeholders.
Due to requests, we have now re-opened our family survey, and once again invite families to share their views and experiences, both positive and negative.
Please submit your responses by June 8, 2025.
By participating in our confidential survey, you will help us to better understand the experiences of families and children who use the CDNTs.
Each survey will take approximately 10 minutes to complete. All data collected is kept confidential and we don’t ask for any personal identifiers.
If you have more than one child attending or waiting for service from a CDNT, please complete one survey per child.
